The Smile Entity went so far as to make Skye believe that she had made peace with her best friend Gemma, but every interaction between them was a very vivid hallucination of the entity.

Their most chaotic and horrific interaction turns out to be a hallucination, but a deleted scene shows what could be Elizabeth and Skye’s final conscious interaction. In it (through Collider), Elizabeth tells Skye she’s going to clear her schedule so they can spend some time off at the Four Seasons, with a spa day included.

Knowing that Skye and Elizabeth’s real and final interaction was aggressive and terrible makes their story even more tragic, as they never managed to repair their relationship before Skye’s shocking deathand that could give the Smile Entity a lot to feed from now that Elizabeth is one of its hosts.

One day, Skye finds her dancers in her apartment, smiling at her, and they attack her, with one of them forcing a giant arm down her throat. Skye wakes up in a wellness clinic with her mother, but it’s not real anymore.
The Smile entity takes control of Elizabeth and she kills herself in front of Skye, but as proven by the end of Smile 2this isn’t real and Elizabeth is alive and well, and Skye was never in the hospital. As mentioned above, Skye and Elizabeth never had a chance to improve their relationship, and now Elizabeth is one of thousands of Smile Entity hosts.
