Cobb stayed behind to finally deal with his memories of Mal and find Saito, who had died in earlier levels and was in danger of being lost forever in limbo. The limbo time expansion with this particular connection was 1 in 160,000, meaning Saito could have already lived his entire life by the time Cobb found him. This is exactly what happened, and it brings Origin a full circle back to where the film began.

Once Cobb finds Saito and reminds him of who he was and the promise he made, it is implied that they kill themselves and wake up. All the other hits in the other levels were successful: the bombs Arthur planned in the elevator, the van falling and hitting the water, and the timer reaching zero on the plane. Origin ends with the entire team waking up in the real world as Saito picks up the phone to make sure Cobb can return to the United States.
