The long-awaited sequel Pathological 2 was announced and is scheduled for release in 2025. Pathological gained a cult following when it was first released in 2005 for bringing a completely different type of horror to players. Developed by Russian company Buka Entertainment and localized into English by G2 Games and GMX Games, the game evoked feelings of existential dread and posed moral and philosophical conundrums.
Pathological 2 was supposed to be a remaster of the original game, but as development progressed its scope grew. When it released in 2019, it was a mix of a remaster and a new game, with only one chapter from the first game available to new players. Now, Pathological 3 appeared on Steampromising to continue the story five years later than its predecessor.

Pathological tells the story of a strange city engulfed by a deadly plague. Players can choose between three characters: the Bachelor, the Haruspex, and the Changeling. each with their own personality, approach to the problem and their role in history. A bizarre plot must be drawn from the town’s strange inhabitants, an experience that may leave players scratching their heads in confusion. The third installment in the series seems to be trying to find a middle ground between the original vision of the game and making it more accessible to new players.

The announcement trailer (available to watch on Steam) shows a man looking at a projector displaying a series of strange and unrelated videos, including a hanging man, children playing, a burning city, an ominous structure, a partially naked woman, and other dark images. The video then cuts to a woman being burned at the stake. The video is accompanied by the text: “…yes, but it will happen yesterday. …and it already happened tomorrow. No, it wasn’t like that at all..
The game will tell the story of a young doctor Daniil Dankovsky, aka Bachelor from PathologicalBut with the ability to travel through time and change the course of events. Daniel must find a cure for the plague, escape the condemnation of the townspeople, and save or doom the people around him – all in 12 days. Pathological 3 It looks like it will continue the strange and creepy atmosphere of the previous two games with new mechanics and a new story.
Morality is more of a philosophical experience than a game

Pathological was far from a perfect game, with clunky controls, lengthy dialogue, and brutal survival mechanics. In addition to the usual hunger and exhaustion meters that are staples of the survival genre, players are also tasked with maintaining thirst, infection, and reputation levels. Combat is difficult and unintuitive and is best avoided at all costs. This game is not easy to play, but this brutal difficulty level is by design. The world depicted in Pathological as relentless as the game.
While this level of difficulty is a necessary part of the game, it’s definitely not for everyone. The original game was described as incredibly important, but not fun to play. The third game in the series looks just as strange as the previous two. This time around, however, I’m hoping to make it at least a little more accessible to the general public, making it more playable without sacrificing all the elements that made it unique. There is no official release date yet, but Pathological 3 will be released on Steam sometime in 2025.
