We the Intellect Devourer
We are the Intellect Devourer begging for help at the beginning of Act 1 of Baldur’s Gate 3 while trying to navigate and escape the Nautiloid. They can become a familiar in Act 2 if they do not die before the Nautiloid falls. To obtain them as minions, four conditions must be met during this first meeting in Act 1:

Connor Vinderblad the Zombie
Connor Vinderblad is Mayrina’s deceased husband, found in Sunlit Wetlands in Act 1. After defeating Aunt Ethel, find and take her wand, Bitter Divorce, and use it to reanimate Connor’s corpse behind the teahouse.
If you decide to keep the wand instead of giving it to Mayrina, you can use it to summon Connor at will as an action. He will stay and fight for ten turns before needing to be summoned again. Note that this only works if Mayrina does not give herself to Aunt Ethel; she survives the encounter, and the group talks to her before using the wand. Failure to meet any of these conditions causes Connor to reappear as a hostile enemy and be killed by the party; he cannot be resurrected again, as the wand will disappear from the party’s inventory.
Servants of the Spore
Various creatures and monsters from Acts 1 and 2 of Baldur’s Gate 3 can become servants of Spore. To get a Spore Servant, you must use Sovereign Glut’s Animating Spores appear on any creature’s corpse and keep it as a minion for as long as the party and Sovereign Glut are in the Underdark. He Who Was has a pet Raven named Quothe, and you can keep him as a permanent family member through Summon Quothe the Raven spell. This spell is only available if you pick up the Raven Gloves at the eastern end of the Shadow Cursed Lands in Act 2, helping He Who Was with his side quest, called Punish the wicked.
To get the Raven Gloves, you must find Ledger in The Waning Moon tavern and return it to him. So when the option is presented, either tell Madeline to stab herself just once or call her a coward. Having Madeline stab herself more than once will cause He Who Was to become angry and attack, and looting her body afterwards will not yield the Raven Gloves.
Quothe the Raven can fly, which makes them a good familiar for exploring dangerous areas. They can also attack an enemy’s eyes, blinding them for two turns.
Exchange
Using the Planar Ally: Cambion Infernal Rapier spell given to Wyll by Mizora, you can have a Permanent Cambion ally once per Long Rest. To obtain the Infernal Rapier and the Cambion ally that comes with it, you must free Zariel’s Active from the Mindflayer Colony after defeating Ketheric Thorm in Act 2. Carne, brother of Balthazar
Within the Gauntlet of Shar, Balthazar enlisted the aid of a Flesh Golem servant made partially from pieces of his unborn twin brother. When speaking to Balthazar, if you ask about Meat and then ask for supplies, and you succeed on a DC 14 Persuasion check, you will receive the Summon Golem Bell. You can get a permanent Deva companion using Views of the Seelie: Summon Devaa spell unlocked in a book in the Vault of Sorcerous Sundries in Baldur’s Gate. To access this powerful minion with the Revivify ability, you will need to break into the Sorcerer’s Vault below Miscellaneous Sorcerers, taking the room key from Tolna Tome-Monger and solving the Vault’s puzzle. You must walk north through the illusory wall west of the Portal within the Vault to access a hidden room containing Journals, Letters, and, on the shelf, a mysteriously glowing green book called Beware before the Seelie.
Reading the book adds Scroll of Bestial Communion to your inventory. It is the only one that exists in Baldur’s Gate 3. Although it can be used once to cast Views of the Seelie: Summon Devayou must have Gale or another Wizard character write the spell to get the Deva as a permanent companion for the remainder of the game. You can then cast the spell once per short rest using a level 6 spell slot.
