There is no guarantee that Harry will start Resident Foreigner Season 4 as part of the story. He finished Resident Foreigner Season 3 Trapped Aboard the Gray Aliens Star Wars-esque moon ship, so he'll need to find a way to escape before he can return to Earth, let alone Patience. The same episode that revealed Harry's incarceration also teased the possibility that the Mantid would at least try to live like Harry for a while. Then, Resident Foreigner Season 4's new villain could serve as the town's doctor for a while, which would certainly be interesting.
Furthermore, one of the main reasons Harry continued to be Patience's doctor was to help him blend in. When he inevitably escapes the Greys, he will return to a town that virtually all of the main characters now know for sure. that aliens exist, and they will be focused on taking down the nefarious extraterrestrials from orbit. Therefore, Harry will no longer need the camouflage of human employmentand he will also have much higher priorities in Resident Foreigner Season 4 than treating the city's residents.
